What Is the Pelvic Floor (and what does it do)?

The pelvic floor is a sling (or hammock) of muscles that sits at the bottom of your pelvis. It attaches at four key points:

  • Pubic joint (front)
  • Tailbone/coccyx (back)
  • Left sit bone (ischial tuberosity) (side)
  • Right sit bone (ischial tuberosity) (side)

It’s not one muscle—it’s multiple layers of muscles, typically described as three layers, each with a main “job”:

The pelvic floor helps with:

  • Continence: helps prevent leaking urine, stool, or gas—especially during effort (laughing, coughing, sneezing, jumping, lifting)
  • Organ support: supports the bladder, uterus, and rectum (important for prolapse symptoms like heaviness or bulging)
  • Sexual function: contributes to comfort, sensation, and arousal
  • Core foundation: it’s the base of your core, working with your deep abs and back muscles to stabilize you
  • Pressure + breath synergy (“core canister”): it works in synergy with the diaphragm to manage intra-abdominal pressure
  • “Sump pump” effect: as it moves with breathing, it supports circulation/lymph flow in the pelvic region 

What Causes Pelvic Floor Dysfunction?

Pelvic floor dysfunction happens when the system can’t coordinate pressure and support effectively—often because the pelvic floor is too weak, too tight, slow to respond, or not syncing with breath and core.

Common causes include:

👉 Pregnancy & birth
👉 Hormonal shifts (peri/menopause)
👉 Pressure overload
👉 Posture & movement patterns
👉 Surgery & injury
👉 Stress & guarding
👉 Connective tissue factors

Women who have pelvic floor dysfunction may experience urine leaking when they laugh, cough, sneeze, jump, run, or lift—especially during exertion (often called stress urinary incontinence). Others notice urinary urgency (the sudden “I have to go right now” feeling), urge leakage, or classic triggers like “key-in-the-door” syndrome, hearing running water, or seeing a toilet.

Some women find they’re peeing more often than normal (commonly more frequently than every 2.5–4 hours), waking at night to go, or feeling like they can’t fully empty their bladder. Pelvic floor dysfunction can also show up as bowel changes, including constipation/straining, difficulty controlling gas, or anal incontinence (leakage of stool).

Another common presentation is heaviness, pressure, or a bulging sensation in the vagina—sometimes feeling like “something is there” or “falling out”—which can be associated with pelvic organ prolapse symptoms. Women may also experience painful intimacy (dyspareunia), vaginal dryness/irritation, or discomfort that makes sex feel unpleasant or even impossible.

It can also contribute to low back pain or pelvic girdle/hip pain, especially when core work and typical strengthening routines don’t seem to help. Many women silently manage these symptoms for years due to embarrassment or being told it’s “normal,” even though effective support and safe, progressive training options exist.

They go from being carefree to mapping the nearest bathroom before they even leave the house.

From jumping into a workout class for fun to skipping movement altogether—because leaking, heaviness, or urgency is too stressful to risk in public.

From a normal drive across town to white-knuckling the whole trip, scanning for gas stations and exits “just in case.”

From sleeping through the night to waking up multiple times to pee, then dragging through the next day exhausted and inflamed.

In fact, incontinence is consistently cited as a leading reason families seek nursing home care—not because it’s “just leaking,” but because it’s exhausting, limiting, and often paired with preventable decline.

Hi, I’m Kim Vopni AKA The Vagina Coach…

My journey with pelvic health and fitness didn’t start with me.

When I was young, I saw my mother struggle with incontinence.

She shared with me her frustration, her worry, and I watched as it affected her life. She started to limit the activities she was doing.

She stopped running, she stopped going for long hikes or doing group activities...

I watched her struggle for years.

Until one day she told me she was scheduled for surgery - a bladder sling for her incontinence…

I grew up not wanting to have kids because of how it affected my mom’s life but I changed my mind after marrying my husband and witnessing my sister in law’s birth.

The following year I was pregnant and asked my midwives about how to prevent tearing. They recommended a product to me called the EPI-NO and that was the catalyst that brought me to the work I do today.

In my mind, if I didn’t tear, I would be fine. My mom had episiotomies and she struggled so if I could avoid that I thought I would be in the clear.

I didn’t tear, in either of my births so you can imagine how shocked I was when…

I thought, “But I didn’t tear and I did kegels - what am I missing?”

I had started my business selling the EPI-NO in Canada and was getting a fast-tracked learning about all things pelvic floor. It uncovered A LOT I didn’t know!

What stood out was that we could apply fitness principles to the group of muscles called the pelvic floor. Kegels had some success but weren’t training the pelvic floor dynamically.

Once I took a whole body approach, I was leak free in a few weeks.

Then I started helping my clients who were struggling with the same challenges… And they reversed their symptoms, too!
